This position requires a CDL-B with passenger and air brake endorsements with starting pay of $14.00/hr. Operating hours are from 5:30 am to 7:00 pm with variations by shift and program. Community Health Nurse - Assists with the coordination and delivery of health care related services to individuals and their families in the Greater Franklin area. Focus is on home visits, teaching and coaching to promote health and wellness and disease prevention. Work is carried out as part of a team process with Care Coordinator and Long Term Support Counselor. Baccalaureate degree in Nursing and an active license in the State of New Hampshire required, experience addressing health needs of seniors, skill in collaborating with individuals and their families, area health care providers and local agencies to ensure proper coordination of care. Assistant Site Manager - Part-time position (2-3 days per week) for Family Planning in Laconia. Responsible for day-to-day management of busy clinic environment including medical records management, patient and insurance billing systems, patient education, appointment scheduling and general oversight of facility. Previous experience in reproductive health care services working with low-income women and teens. Medical billing experience required. APRN for Family Planning - located at the Lakes Region Family Center. Responsibilities include provision of comprehensive family planning services and oversight of program service implementation in a Title X funded Family Planning setting. Must have reproductive health care experience and be able to provide comprehensive services including STD testing for men and women. Previous experience in a community/preventive health program working with a diverse population is required. Position requires current New Hampshire licensure. Position is 25 hours per week.
